Your autonomy, dignity and safety matter. Texas law requires that I share the following information so that you are aware of your rights and options available to you as consumer of mental health services.

You have the right to request access to your health care records at any time. If you would like to request your records you may submit a written request via email to your therapist. All requests are handled with care and in accordance with state and federal laws. Feel free to reach out with any questions or assistance with this process.

If at any time you feel that you have not received that or have concerns about your care, we ask that you give us the opportunity to discuss this with you. If you are not satisfied with the outcome, you have the option to file a complaint with the Office of the Texas Attorney General’s Consumer Protection Division.
